Why this happens

Pantoprazole can slow down the way your liver processes acenocoumarol. This leads to higher levels of the blood thinner staying in your system for longer.

You must inform your doctor or anticoagulant clinic if you start, stop, or change your dose of pantoprazole. They may need to perform more frequent blood tests (INR tests) to check how quickly your blood clots and may adjust your acenocoumarol dose accordingly. Seek medical help immediately if you notice unusual bleeding, such as nosebleeds, bleeding gums, or dark, tarry stools.
