Can I take Allopurinol and Metformin together?

There is a potential for an increased risk of a rare but serious condition called lactic acidosis when taking these two medicines together, particularly if your kidney function is reduced. Allopurinol may also slightly increase the levels of metformin in your blood.

Why this happens

Both allopurinol and metformin are cleared from the body by the kidneys. If the kidneys are not working at 100%, allopurinol can compete with metformin for removal, or the combination can put extra strain on kidney function. This can lead to a buildup of metformin, which increases the risk of lactic acidosis (a buildup of acid in the blood).

Most patients can take these medicines together safely, but it is important to have your kidney function checked regularly by your doctor (usually via a blood test). Seek immediate medical help if you experience signs of lactic acidosis, such as extreme tiredness, muscle pain, difficulty breathing, or feeling unusually cold.
