Taking certain antihistamines with ramipril may increase the risk of a rare but serious allergic reaction called angioedema (swelling under the skin). Additionally, some older antihistamines can cause drowsiness, which may be worsened if ramipril lowers your blood pressure and makes you feel dizzy.

Why this happens
Ramipril is an ACE inhibitor that prevents the breakdown of certain substances in the body (like bradykinin). Some antihistamines, particularly those used for hay fever or hives, might interact with the same pathways, potentially triggering sudden swelling of the face, lips, or throat. Furthermore, both drugs can have a combined effect on your alertness if the ramipril causes a drop in blood pressure.
