Can I take Gabapentin and Modafinil together?

Drug interaction guide

Taking gabapentin and modafinil together may lead to increased side effects such as dizziness, drowsiness, or difficulty concentrating. While modafinil is a stimulant and gabapentin is often used for nerve pain or seizures (and can cause sedation), they do not necessarily 'cancel each other out'. Instead, you might feel a confusing mix of being alert yet dizzy or uncoordinated.

Why this happens

There is no significant evidence that these drugs change how the other is absorbed or cleared by the body (pharmacokinetics). However, they both affect the central nervous system in opposing ways (pharmacodynamics). This can lead to unpredictable effects on your coordination, alertness, and mental clarity.

You can usually take these together, but you should be cautious, especially when first starting the combination or changing your dose. Do not drive or operate heavy machinery until you know how the mix affects you. If you feel excessively dizzy, confused, or 'spaced out', contact your doctor.

Breathing problems or opioid use

Gabapentin can cause serious, life-threatening breathing difficulties, especially if you already have lung disease or if you take it alongside opioid pain relief (such as morphine or codeine).

Mental health disorders

A small number of people treated with gabapentin have had thoughts of harming or killing themselves. You must seek immediate medical help if you notice any changes in your mood or suicidal thoughts.

Abrupt discontinuation

Do not stop taking gabapentin suddenly. Doing so can cause serious side effects, including an increase in seizure frequency or severe withdrawal symptoms. Your dose must be reduced gradually.

Contraception and Pregnancy

Modafinil reduces the effectiveness of hormonal contraceptives (the pill, patches, or implants). You must use an additional barrier method, such as condoms, during treatment and for two months after stopping. It must not be used during pregnancy as it may cause harm to the unborn baby.

Severe skin reactions

Stop taking modafinil and seek urgent medical attention if you develop a skin rash, hives, or blisters, as it can cause rare but life-threatening skin reactions.

Alcohol

It is best to avoid or strictly limit alcohol while taking gabapentin. Combining alcohol with gabapentin can significantly increase the risk of severe drowsiness, dizziness, and breathing difficulties. It may also impair your coordination and judgment more than usual.

It is best to avoid or strictly limit alcohol while taking modafinil. Combining the two can increase the risk of side effects like dizziness, drowsiness, and impaired judgement. Additionally, modafinil can mask the effects of alcohol, making you feel less intoxicated than you actually are, which can lead to dangerous situations.

Caffeine (Coffee, tea, energy drinks)

Be cautious with caffeine intake. Both modafinil and caffeine are stimulants; combining them can increase the risk of side effects like jitteriness, a racing heart, anxiety, and high blood pressure.
