Can I take Losartan and Rybelsus together?

Why this happens

Losartan (an ACE inhibitor) can increase sensitivity to insulin, which may enhance the blood-sugar-lowering effects of Rybelsus (semaglutide). Both medications can also influence blood pressure and kidney filtration.

Monitor your blood sugar levels regularly, especially when starting or changing doses. Stay well-hydrated. If you experience symptoms of low blood sugar (such as shakiness, sweating, or confusion) or notice a significant change in how much you urinate, contact your doctor. Your doctor may need to adjust your dosages.

Pregnancy

You must not take losartan if you are pregnant. It can cause serious harm or death to an unborn baby, especially if taken during the second and third trimesters.

Pancreatitis

Stop taking Rybelsus and seek immediate medical help if you experience severe, persistent stomach pain (which may reach through to your back), as this can be a sign of an inflamed pancreas (pancreatitis).

Thyroid cancer

Do not take Rybelsus if you or any of your family have ever had medullary thyroid carcinoma (MTC) or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2). Contact your doctor immediately if you notice a lump or swelling in your neck.

Diabetic eye disease (retinopathy) - Rybelsus

Inform your doctor if you have a history of diabetic retinopathy. Rapid improvements in blood sugar levels can cause a temporary worsening of this condition.

Alcohol

It is best to avoid or strictly limit alcohol while taking losartan. Alcohol can increase the blood pressure-lowering effect of losartan, which may make you feel dizzy, lightheaded, or faint, especially when standing up. Combining alcohol with medication also increases the risk of liver damage.

It is best to avoid or strictly limit alcohol while taking Rybelsus. Both alcohol and Rybelsus can lower your blood sugar levels; combining them increases the risk of severe hypoglycaemia (low blood sugar). Additionally, Rybelsus can cause nausea and vomiting, which alcohol may worsen.

Potassium-rich foods (e.g., bananas, spinach, salt substitutes)

Losartan can increase the levels of potassium in your blood. You should avoid using salt substitutes containing potassium (such as Lo-Salt) and speak to your doctor before significantly increasing your intake of potassium-rich foods, as high potassium levels can affect your heart rhythm.

All food and drink (Timing)

Rybelsus must be taken on an empty stomach first thing in the morning. You must wait at least 30 minutes after taking the tablet before having your first food, drink (other than a small sip of plain water), or any other oral medicines. If you eat or drink too soon, the medicine will not be absorbed properly and will not work.
