Taking these two medicines together can cause your blood pressure to drop too low. This might make you feel dizzy, lightheaded, or faint, especially when you stand up quickly from a sitting or lying position.
Why this happens
Both losartan and trazodone have the effect of lowering blood pressure. Losartan is specifically designed to do this, while trazodone (an antidepressant) can cause low blood pressure as a side effect. When used together, their effects add up.
What you should do
You should monitor your blood pressure regularly at home. Be careful when getting out of bed or standing up; move slowly to give your body time to adjust. If you feel very dizzy, sit or lie down immediately. Inform your doctor if you experience frequent dizziness or fainting, as they may need to adjust your dosages.
Important precautions
Depression and mental health disorders
Trazodone can increase thoughts of self-harm or suicide, particularly when first starting the medicine or changing the dose. Monitor your mood closely and seek urgent medical help if you experience any distressing thoughts.
Serotonin Syndrome
When taken with other medicines that affect serotonin, trazodone can cause a life-threatening condition called serotonin syndrome. Seek immediate medical attention if you experience high fever, agitation, shivering, or muscle twitching.
Priapism (prolonged erection)
Trazodone can cause a painful, persistent erection that lasts for several hours. This is a medical emergency; if this occurs, stop taking the medicine and go to an A&E department immediately to avoid permanent damage.
Pregnancy
You must not take losartan if you are pregnant. It can cause serious harm or death to an unborn baby, especially if taken during the second and third trimesters.
Food and drink warnings
Alcohol
It is best to avoid or strictly limit alcohol while taking these medicines. Alcohol can increase the blood-pressure-lowering effect of losartan, making you feel dizzy or lightheaded. When combined with trazodone, alcohol will significantly increase drowsiness and may impair your coordination and judgement.
Potassium-rich foods (e.g., bananas, spinach, salt substitutes)
Losartan can increase the levels of potassium in your blood. You should avoid using salt substitutes containing potassium (such as Lo-Salt) and speak to your doctor before significantly increasing your intake of potassium-rich foods, as high potassium levels can affect your heart rhythm.
Grapefruit juice
Grapefruit juice may increase the amount of trazodone in your blood, which can increase the risk of side effects. It is best to avoid drinking large amounts of grapefruit juice while taking this medication.
