Taking these two medicines together may increase the risk of a rare but serious allergic-type reaction called angioedema (swelling under the skin). It may also cause your blood pressure to drop lower than intended, potentially making you feel dizzy or faint.
Why this happens
Both drugs can affect a system in the body that regulates swelling and blood pressure. Ramipril (an ACE inhibitor) prevents the breakdown of a substance called bradykinin. Pyridostigmine can also influence this pathway, and when combined, the levels of bradykinin may rise too high, leading to swelling. Additionally, both drugs can lower blood pressure through different pathways, leading to an additive effect.
What you should do
You can usually take these together, but you must be aware of the signs of an allergic reaction. Seek immediate medical help if you experience swelling of the face, lips, tongue, or throat, or if you have difficulty breathing. If you feel very dizzy when standing up, speak to your doctor as your blood pressure may need monitoring.
Important precautions
Angioedema (Swelling)
Seek immediate medical help if you experience swelling of the face, lips, tongue, or throat. Ramipril can cause a rare but serious allergic reaction called angioedema, which can block your airway.
Asthma or COPD
Use with extreme caution if you have asthma or chronic obstructive pulmonary disease (COPD). Pyridostigmine can cause narrowing of the airways and increase secretions, which may severely worsen breathing difficulties.
Pregnancy
Do not take ramipril if you are pregnant. It can cause serious harm to an unborn baby, particularly affecting their kidney development. Use effective contraception while taking this medicine.
Cholinergic Crisis (Overdose) - pyridostigmine
If you experience severe muscle weakness, abdominal cramps, or difficulty breathing, seek urgent medical help. These can be signs of an overdose, which can look very similar to the condition being treated.
Food and drink warnings
Alcohol
It is best to avoid or strictly limit alcohol while taking pyridostigmine. Combining alcohol with this medication can increase the risk of side effects such as dizziness, drowsiness, and blurred vision. Alcohol may also worsen the symptoms of the condition for which you are taking pyridostigmine.
It is best to avoid or strictly limit alcohol while taking ramipril. Alcohol can increase the blood pressure-lowering effect of ramipril, which may make you feel dizzy, lightheaded, or faint, especially when you first start the medicine or move to a higher dose.
Salt substitutes (Potassium-based)
Avoid using salt substitutes that contain potassium (such as Lo-Salt). Ramipril can increase the levels of potassium in your blood. Using these substitutes alongside your medicine can cause your potassium levels to become dangerously high.

Can I take Maitake and Turkey tail together?
There are no known harmful interactions between maitake and turkey tail mushrooms. Both are medicinal fungi that are often taken together to support the immune system. However, because both can stimulate the immune system, there is a theoretical possibility of an additive effect.
