Can I take Pyridostigmine and Ramipril together?

Taking these two medicines together may increase the risk of a rare but serious allergic-type reaction called angioedema (swelling under the skin). It may also cause your blood pressure to drop lower than intended, potentially making you feel dizzy or faint.

Why this happens

Both drugs can affect a system in the body that regulates swelling and blood pressure. Ramipril (an ACE inhibitor) prevents the breakdown of a substance called bradykinin. Pyridostigmine can also influence this pathway, and when combined, the levels of bradykinin may rise too high, leading to swelling. Additionally, both drugs can lower blood pressure through different pathways, leading to an additive effect.

What you should do

You can usually take these together, but you must be aware of the signs of an allergic reaction. Seek immediate medical help if you experience swelling of the face, lips, tongue, or throat, or if you have difficulty breathing. If you feel very dizzy when standing up, speak to your doctor as your blood pressure may need monitoring.
